FocusAbility provides person-centred care and support for adults with learning disabilities, autism and a range of additional support needs across Lincoln and the surrounding areas.
Our aim is to help the people we support live full and meaningful lives, with as much choice, control and independence as possible.
Support can be provided in an individual’s own home or tenancy and tailored around their individual circumstances, preferences and support needs.
Community Supported Living
Our Community Supported Living service helps people live more independently while receiving the care and support they need.
Rather than taking a one-size-fits-all approach, we work around the individual. Support can range from a few hours a day through to around-the-clock care, depending on their needs.
FocusAbility is registered on the Lincolnshire Adult Social Care Provider Framework for Community Supported Living and is also registered with the Care Quality Commission (CQC).
Support in Your Own Home
FocusAbility is a domiciliary care provider, supporting people both in their own homes and within their own tenancies.
We understand that being able to live in your own home can be an important part of maintaining independence.
Our support is planned around the individual, helping them to have greater choice and control over their everyday life while having appropriate support available when they need it.
Community & Floating Support
Our community support is designed to help individuals participate in everyday life and become more independent within their local community.
Depending on the individual’s needs and person-centred support plan, FocusAbility can provide support with everyday activities, accessing the community and attending appointments.
The aim is not simply to do things for somebody, but to provide appropriate support that helps each person achieve the best possible outcomes and live as independently as possible.
Choice, Control & Independence
Everyone has different abilities, goals and support needs.
That’s why FocusAbility takes a person-centred approach, creating support around the individual rather than expecting the individual to fit around the service.
Our approach focuses on helping people have maximum choice and control over their lives while providing support in the least restrictive way possible.
This could mean supporting someone to become more independent at home, access their local community, attend appointments or simply providing the additional care and reassurance they need in everyday life.
Support Built Around the Individual
The level of support someone needs can change considerably from one person to another.
FocusAbility provides planned and coordinated person-centred support appropriate to the individual, with the aim of helping people live fulfilling lives and achieve positive outcomes.
Whether someone requires support for a few hours or more comprehensive ongoing care, we’ll work to understand their individual needs and the outcomes that matter to them.
